FromSoftware games are notorious for their multiple and sometimes very obscure endings, and Sekiro: Shadows Die Twice is no different. This page will detail how to achieve and unlock all possible endings such as the Sekiro Purification Ending, Shura Ending, and even more.
HUGE SPOILERS AHEAD (Obviously)

Video Guide: Shura Ending and Boss Fight Walkthroughs
This is the fastest ending in Sekiro: Shadows Die Twice, but also the least rewarding
Progressing through the game, you'll be prompted to return to Ashina Castle with everything you need to end your lord Kuro's immortality. All of the Sculptor's Idols will no longer work, and you'll find your father Owl awaits you on the rooftop. After speaking with him, he will give you the choice to obey him or Kuro. Choosing the option to obey Owl will trigger this ending. You'll fight two exclusive bosses to this ending back to back, but it cuts out the last region and several more sequences that you'll miss out on if you are choosing this early end.
See the Shura Ending Walkthrough for more details and how to take on the bosses you'll face in this path.
The next three endings all come from choosing to side with Lord Kuro against Owl - and in fact you can go about the process of securing all three endings and pick which one you want to experience at the very end.
Video - Immortal Severance
Opposite of the Shura ending, when you reach the decision to choose between your father Owl and your Lord Kuro, side with Kuro. This is the ending most players will likely get. It involves getting the Dragon's Tears from the Divine Realm after exploring the Fountainhead Palace.
Upon your return to Ashina Castle, head to the Silvergrass Field where you lost your first fight to Genichiro, and defeat the boss here - and you'll be able to give Lord Kuro the Dragon's Tears afterward.
Note that going through the side quests for the other two endings will not lock you out of this one - you'll be able to choose between all three if you've met the criteria.

Video - Purification Ending
While this questline can be started before or after facing Owl, it must be started before you defeat the Divine Dragon of the Fountainhead Palace.
This ending is slightly better than the Immortal Severance Ending. When presented with the choice between Owl and Kuro, again choose Kuro. After doing this you'll follow these steps.
- When in Lord Kuro's Room and he is standing near the incense burner (this can be before or after the attack on Ashina Castle), you can eavesdrop on Kuro by standing behind the screens at the back of the room.
- Rest, and then speak to Emma, who has moved up to the Castle Tower Lookout. Choose the option to agree that Lord Kuro cannot be allowed to die. This is as far as you can go before making the choice with Owl.
- Defeat Owl on your return to Ashina Castle (Dusk) after obtaining the Immortal Blade, Lotus of the Palace, and Shelter Stone.
- Rest, and speak to Emma after telling her that Kuro cannot die, and this time she will give you Tomoe's Note, and learn of the path of Purification.
- Warp and speak to Emma again near the Old Grave Sculptor's Idol, on the other side of the small building.
- Head back to Dilapidated Temple, where Emma is with the Sculptor. The sculptor mentions the room is drafty, and you can eavesdrop on Emma and the Sculptor from the back of the building near a hole in the wall.
- Speak with Emma and choose the option to ask about what you overhead. Persist and ask her to reveal the truth, and receive Father's Bell Charm from her.
- Use the bell to pray to Buddha. This unlocks a more difficult version of the Hirata Estate - which begins where you met Owl, only now there is a Lone Shadow Mini-Boss you must defeat.
- Use a new shortcut past the Lone Shadow to re-fight Juzou the Drunkard and some tougher enemies as you make your way to the Audience Chamber.
- Heading to the Lady Butterfly boss room, you'll need to defeat Owl again, only he is a much tougher version to face. Defeat him to earn the Aromatic Flower.
- After defeating him, progress through the story as normal until the end fight against Sword Saint Isshin.
- After defeating him, choose to give Kuro the Divine Dragon's Tears and Aromatic Flower.
Video - Return Ending
Like the Purification Questline, this questline can be started early on either before or after facing Owl in Ashina Castle - but it must be completed before you defeat the Divine Dragon Boss of the Fountainhead Palace.
You thought the Purification Ending was convoluted? Well this one has even more steps, though it's worth the work as it seems to be the best ending in the game. Ready? Here we go.
Acquire the Holy Tome: Infested
This item can be gotten in one of two ways depending on how you progressed through the world of Sekiro. If you went to Senpou Temple before beating Genichiro, you'll find the High Priest of Senpou Temple next to the Main Hall Idol who will give you this item.

If you went through Senpou Temple AFTER defeating Genichiro, then you'll need the Mibu Breathing Technique to get it. You'll find it in a pool of water near the Temple Grounds Sculptor's Idol.
The Divine Child and the Holy Chapter: Dragon's Return
Give the Holy Tome: Infested to the Divine Child of Rejuvenating Water who you met after defeating the Folding Screen Monkey's. Next, take rice from the Divine Child until she becomes sick.
You'll need to refresh the area several times in order to get more rice. She also won't give you more rice until you consume the previous rice she gave you. This can also happen by giving the Old Crone rice when she asks for it.
After the Divine Child is sick, give her Persimmons to cure her. If you don't have any head to the Shugendo Idol and purchase some from the Memorial Mob there. After curing the Divine Child with Persimmons, she will give you some Rice for Kuro, a specific Key Item. Head to your Lord and give it to him.
Be sure to rest and wait until he gives you Sweet Rice Ball specifically made from the Divine Child's rice (you can eat it in front of him or not - it doesn't matter).
Heading back to the Inner Sanctum Idol, speak with her to confirm you gave him the rice and that he made a treat from it, and then rest. When you rest again, she will be facing the shrine and speaking aloud to the other Divine Children.
At this point, you must have give her the Holy Tome: Infested to the Divine Child to advance this questline further.
When next you rest (and she's read the Holy Tome: Infested), You'll find she's left the Inner Sanctum entirely.
In fact, she's moved to the Halls of Illusion - the boss area of the Folding Screen Monkeys that you can access near the Inner Sanctum. She'll ask you to find the High Priest of Senpou Temple, who is said to reside in a tunnel.
You'll find his corpse in the cave outside the Main Hall that leads to the pagoda on the cliff - in a room with several Centipede Assassins (he's the only monk corpse wearing purple). Search the corpse for the Holy Chapter: Dragon's Return, and bring back the scroll to the Divine Child back in the Halls of Illusion.
Return to the Inner Sanctum and rest, and she'll then tell you she needs two fruits of the Great Serpent.

How to get the Fresh Serpent Viscera
This step is best completed if you've also completed the first step of Blackhat Badger's questline. However, this isn't necessary.
From the Sculptor's Idol head through the open door and toward the bridge. Going to the area below this, you'll see a Senpou Assassin standing by the controls of a kite. Deathblow him with stealth from behind, then use the Ninjutsu Puppeteer ability to get him to raise the kite.
With the kite raised, make your way to it. Head up to the first temple with three monks standing outside it. Head through the open window of the temple and use your grappling hook to get to the top of the large tree there. Grappling to the kite, you can cross the valley. Once on the other side, you can travel along the cliff edge and eventually find a cave with a Sculptor's Idol. Continue through the cave to reach an overhang above the Great Serpent.
You can probably take it from here, but we'll walk you through it. Jump off the ledge and use a plunging deathblow on the Great Serpent, which will trigger a second deathblow that will finally put an end to the problematic python.
How to Get the Dried Serpent Viscera
The first step to finding this item is heading into the bowels of the Sunken Valley. From the Riven Cave Sculptor's Idol you can head straight out and make your way down to the poison lakes down below in the valley. You should be able to grapple to praying hands and trees as you make your way down, avoiding the fall damage.
Next you'll want to find the Toxic Memorial Mob Merchant. There will be several enemies in the area (damned monkeys), but once you find this merchant, you're heading in the right direction. Head into the cave near him.
Inside this cave you'll find a massive snake, while the body lies all along the cave, the head is guarding a small shrine, which we need to get to. Follow the path along the right hand side of the cave, taking care to avoid the snake's body, and not hit it with your sword while attacking any enemies nearby.
Once getting past the body, you'll arrive at the head guarding the shrine. If you try to run across the bridge, you'll be met with a snappy death. Look to your left, where you'll find a small hole he won't bother you about.
Hop down to the ledge below and follow the path up to the terrified monkey. Use a backstab Deathblow on him, followed by your Puppeteer Ninjutsu ability. This will create a distraction for the massive snake. Use the grapple point nearby to swing to a ledge, and hurry to the building behind him, where you'll find the Dried Serpent Viscera in the hands of a shrine.

Return to the Divine Child
After you have both Viscera in hand, return to the Inner Sanctum and give the two fruits to the Divine Child.
Rest once, and you'll find the door closed - and you can eavesdrop to hear the Divine Child writhing in pain. Rest one more time after eavesdropping and the doors will open once more.
Note that if resting doesn't reset her to be where you can speak with her, you may need to defeat the first boss of the Fountainhead Palace to progress, the Corrupted Monk - True Monk.
Once the doors are open, speak to her and she'll give you an item called Frozen Tears. After you have this item, progress through the game as normal and at the end, give Kuro the Frozen Tears and Divine Dragon Tears to achieve this ending.
